Quality Improvement/Audit Tools

CHC developed a QA Manual that was revised in October 2011. The manual is a comprehensive program with tools that may utilized independently; resources are based on systems and cross multiple departments within the organization... The manual is available in 3-ring binder with disks or individualized components may be sent by e-mail. Other audit tools and/or tracking are developed based on survey trends [nationally and in Virginia] and are available at a reasonable cost per audit form.

Resource Manuals

We have developed resource manuals on Accident Prevention and Management, Urinary Incontinence; Housekeeping/Laundry Services; Nutrition/Hydration Resource for interdisciplinary team, Dietary Sanitation, and Care Plan Resources that are a compilation of resources intended to provide the nursing facility with the necessary education and tools to develop and sustain an effective program for preventing, identifying, treating, monitoring and responding to areas of risk. Adoption of any policy, procedures, and/or tools should be carefully reviewed by the facility's clinical management team and medical director prior to implementation.
